Put yourself in the enrolling physician&#39;s chair. A patient in second remission is in front of you and there&#39;s a maintenance trial available. Before anything else, you ask yourself one question: do I believe I can get this patient to a transplantable state? 
 
If the answer is yes, or even maybe, you don&#39;t enroll them. You pursue donor identification. You optimize their comorbidities. You get them toward the only potentially curative option that exists in this disease. You do not randomize a patient away from a possible cure and into an investigational maintenance arm because you thought it&#39;d be interesting.  
 
Allo-HCT in CR2 offers long-term survival nothing else in this setting comes close to (yet :D). It&#39;s an obligation. The textbooks call this clinical equipoise, the ethical requirement that you genuinely believe neither arm is clearly better for this specific patient. No hematologist is in equipoise about a patient they think can reach transplant. So the question answers itself, and the answer is a filter. 
 
Which means REGAL&#39;s enrollment was never a dice roll. Physicians at all the enrollment sites each made that judgment, patient by patient, before randomization. The literature calls it channeling bias or enrollment gatekeeping.  
 
And that judgment usually holds, because the reasons are structural, not transient. Age. Comorbidity burden. Organ dysfunction. No suitable donor (though less of an issue now). None of that resolves over a few months of maintenance therapy. Those doors don&#39;t tend to reopen. 
 
Is the crossover rate zero? No. Let&#39;s look at QUAZAR AML-001, the closest precedent we have. In QUAZAR, a maintenance trial in transplant-ineligible AML patients, roughly 13.7% of the placebo arm eventually received an allo-transplant. So no, the ineligible label doesn&#39;t hold forever. It held at randomization, only 6 patients out of 472 went to transplant while still in first remission, which tells you the enrolling physicians&#39; initial judgment was sound. But circumstances changed for a meaningful minority downstream in that those patients relapsed, got re-induced, and then became transplant candidates. 
 
However QUAZAR enrolled patients in CR1. A CR1 patient who relapses can be re-induced into CR2 and reconsidered for transplant. That&#39;s a well-trodden path. REGAL patients are already in CR2. If they relapse, they&#39;d need to achieve CR3 to become a transplant candidate, which is a far rarer, harder, worse-prognosis proposition. The escape hatch that opened for 13.7% of QUAZAR&#39;s control arm is structurally narrower one line deeper into the disease. 
 
So I&#39;d expect REGAL&#39;s crossover to run below QUAZAR&#39;s, because there&#39;s simply less runway left to get someone to a transplantable state from second remission than from first. 
 
Overall, the risk is small, and it&#39;s small by design, because the ethics of enrollment filtered for it before randomization ever occurred. Small enough that it can&#39;t carry the &quot;durable BAT subgroup&quot; thesis on its own.

REGAL CRITERIA: Must not be candidates at the time of study entry for Allo-SCT due to intercurrent medical conditions, patient&#39;s preference or lack of an available donor. 
 
Concern of post-trial Rx imbalance impacting REGAL OUTCOME are INVALID:  
 
1) AML CR2 SCT eligibility is a SERIOUS LIFE/DEATH &amp; NOT whimsical decision that changes without CAUSE. 
2) REGAL BAT pts receive toxic maintenance regimens leading to cyclic risks of illness/death &amp; thus remaining SCT-ineligible. 
3) REGAL GPS pts who initially responded (~80% immune reponses) &amp; then comes off trial would be most robust/suitable to be SCT candidates given GPS minimal toxicity. 
4) The same MAGICAL THINKING of BAT pts regaining SCT eligibility would apply to GPS non-responders who are treated with BAT-like regimens after they discontinue REGAL trial.

$SLS Great post, I wanted to emphasize that this is the only meaningful risk factor in REGAL.  It is not BAT IRM (it really does not matter to a large degree how low or high BAT IRM is), favorability mix has much less of an impact than what I thought it would have as well (even up to 60% favorability is not an issue), it is simply the amount of BAT/control that bridges to transplant. 
 
I&#39;ve attached the actual fits.  Even at a BAT IRM of 16 with 30% favorability (essentially 30% of BAT having an 18 to 26 mOS) and 25% transplant, P(success) is 80%, with an HR of .446, and 34 GPS alive / 12 BAT alive by the 80th (of course, before 3% to 10% censoring). 
 
Even at 35% transplant rate for BAT/control, P(success) is 70%.  That is the margin of safety. 
 
In all my research, the likely range across any combination of studies always arrives at 10% to 13% being likely, with a black-swan scenario of 18%.  If anyone discovers anything higher, please reach out to me, but I haven&#39;t seen anything yet
