Re: GSK & Ionis' Bepirovirsen today, this should be viewed positively by $DTIL fans. FDA approved bepirovirsen based on sustained HBsAg loss as an endpoint. Precedent now set. Precision has reported that 100% of patients treated with PBGENE-HBV who had detectable baseline pgRNA achieved durable and complete loss of detectable blood pgRNA, along with substantial declines in HBsAg. No pgRNA, big drops in HBsAg, & a 10-log (90%) reduction in cccDNA means that Precision is reporting what the FDA wants to hear. Nov presentation will shed more light here on HBsAg, pgRNA, and cccDNA reduction, especially among higher dose cohorts. Bepirovirsen showing functional cure in 19% - 26% of HBV pop, leaving 74% - 81% without treatment. Bepirovirsen does not touch cccDNA, leaving the door open to HBV reactivation down the line. PB-GENE-HBV doesn't just suppress RNA long enough for the immune system to step in, but it physically eliminates cccDNA so relapse or reactivation cannot occur.
